The Arizona Partnership for Immunization (TAPI)
Sunset Community Health Center is pleased to announce that we have been awarded with the Teen Cloud and the Toddler Cloud. The Dr. Daniel T. Cloud Awards are given to recognize and honor outstanding practices and the brightest stars, with at least 90% immunization coverage rate, by The Arizona Partnership for Immunization (TAPI). Sunset Community Health Center was the only community health center in the State of Arizona to receive both awards this year.

The NCQA Patient-Centered Medical Home
As part of our commitment to helping build healthier communities Sunset Community Health Center is pleased to announce that our local health clinics have been recognize as Patient Centered Medical Home this recognition will help us continue to better serve the needs of our patients and their family.
The medical home model care is a way of saying the patient is the most important person in the health care system. You are at the center of your health care.
Your Sunset Community Health Center, believes this model enhances positive outcomes. Through this model, working together with their health care team, patients are able to achieve their best quality of life.
The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients' experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient's care is overseen by clinician-lead care teams that coordinate treatment across the health care system. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.
"NCQA Patient-Centered Medical Home recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients," said NCQA President Margaret E. O'Kane. "Recognition shows that Sunset Community Health Center has the tools, systems and resources to provide its patients with the right care, at the right me."
Sunset Community Health Center demonstrated the ability to meet the program's key elements, embodying characteristics of the medical home. NCQA standards aligned with the joint principals of the Patients-Centered Medical Home established with the American College of Family Physicians, the American Academic of Pediatrics and the American Osteopathic Association.
